The best breast augmentation surgery is achieved when the shape of the breast and the position of the areolas are corrected in order to feel a beautiful bust when dressed and especially without clothing.

Where are breast implants placed?

For augmentation or placement of implants the position of the areolas must always be corrected because the prosthesis will increase the defect of the breasts. If the areola is sagging the prosthesis will make it look more sagging and if it is lateralized the problem will be increased. That is why the best result is obtained if the position of the areola is simultaneously corrected. This cannot be corrected if placed via the axillary route, or through the navel. Silicone prostheses or silicone implants, saline solution prostheses or water-filled ones, and hydrogel-filled prostheses or a type of sugar. Silicone prostheses currently have no restriction and time has shown that they are the safest.

 

What is encapsulation? Why does it occur?

 

To prevent the prostheses from becoming hard or encapsulating, pores or microporosities are used in the silicone cover. This type of prosthesis for breast augmentation is what is called a textured prosthesis. The ideal is to place the prosthesis behind the pectoral muscle as they encapsulate less there.

 

When can Pexy be performed without using a prosthesis?

 

When the patient has breasts with sagging areolas but the volume of the breast is large. The surgeon can use various techniques, such as the one called periareolar or only cutting around the areola if the descent is only 2 to 3 cm, or a Colombina-shaped technique when the descent of the areolas is greater than 5 cm. This Colombina-shaped scar allows more adjustment of the skin of the lower part of the breast whereby the shape of the breast is more beautiful. The

vertical scar tends to disappear over time.

 

When should Mastopexy with Prosthesis be performed?

When the areola is sagging but the volume of gland that the patient has is very small. In those cases if the gland is lifted and no filler is placed the breast will be very small. Many times those breasts have the appearance of being very large because they look long and sagging, but the amount of gland is minimal. In those cases a prosthesis must always be used to achieve the volume the patient desires. The incision, just as in the pexy technique without prosthesis, can be periareolar or Colombina-shaped depending also on how many centimeters the areola needs to be lifted.

When is a reduction performed?

When the breast is truly of large volume or cup size and is also sagging and elongated a reduction must be performed.

 

In this case the incisions are always larger:

 

It can be a Colombina, an L or an inverted T. The Colombina is used if the breasts are not very large. But if the patient already has a large size one can consider the L or the inverted T which give a very beautiful shape. The scars require special postoperative care so that they diminish over time.
